Counter-Pattern Misfire & User-Harm Response

Purpose
To govern the immediate response when a deployed counter-pattern overruns its bound and harms the users it was meant to protect, and to any incident of measurable user harm across all THE RESET operations. The procedure ensures rapid recognition of a misfire, immediate rollback, surface restoration, and escalation to the Steward.
Scope
All counter-pattern misfire and user-harm incidents on instrumented or live surfaces.
In scope:
- Recognition of a misfiring or unbounded counter
- Immediate rollback and surface restoration
- Harm assessment across the affected population
- Escalation, notification, and incident filing
Out of scope:
- Routine counter performance review, which follows SOP Surface Monitoring & Mesh Inspection
- Drill rehearsal of this response, which follows SOP Crisis Drill Conduct & After-Action
Definitions
Misfire
A deployed counter-pattern operating outside its doctrine-bounded effect, with potential to harm users.
Rollback
Immediate withdrawal of a misfiring counter and restoration of the surface to its pre-intervention state.
Harm Assessment
Rapid measurement of the scope and severity of user impact from a misfire.
Responsibilities
Site Safety Officer (Owner)
Owns the procedure, maintains auto-injector stock, and trains all field staff.
First Responder
Recognises reactions, administers the Sentinel Antidote, and summons emergency services.
Surface Monitoring Lead
Ensures every field team carries a stocked, in-date misfire kit.
Procedure
Recognition
- Assess the surface for unbounded counter behaviour, runaway friction, or content steering.
- Identify systemic signs such as mass disengagement errors, lockouts, or wellbeing-signal spikes.
- Treat any systemic user impact as a misfire without delay.
Containment
- Trigger immediate rollback of the misfiring counter from the affected surface.
- Freeze the responsible Injector and isolate the surface.
- Restore the surface to its pre-intervention state and confirm via the Mesh.
Aftercare
- Monitor the surface continuously for secondary effects until stable.
- Hand the incident to the Steward with a timeline of actions taken.
- Recertify the counter, restock the response kit, and file the incident report before close of day.
